Mounjaro activates two natural gut-hormone receptors, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

Slower emptying is helpful for appetite, but it has a side effect on comfort. Food stays in the stomach longer, so the feeling of fullness lasts. Fat is the nutrient that naturally takes longest to digest. When a fat-rich meal meets a slower stomach, the combination can feel like a weight sitting under your ribs.

This is also why large portions of any food can be uncomfortable. The stomach is no longer clearing at the pace you were used to, so a plate that felt normal before treatment can now be too much. Smaller, slower meals suit the way the medicine works.

Appetite changes are part of the design. You may find you simply cannot finish a rich meal you once enjoyed, and that is a normal response rather than a sign anything is wrong. Eat until you are comfortably satisfied, not until the plate is empty.

Many people notice a problem with very fatty, fried or very sugary food. Within that, some situations come up again and again.

Takeaways and fast food. Burgers, fried chicken, kebabs and chips combine heavy fat with large portions. If you want a takeaway, choose a smaller size, skip the extras and eat slowly. Stop at the first sign of fullness.

Fried breakfasts and pastries. A greasy fry-up or buttery pastry first thing can set off nausea or reflux, particularly in the days after an injection when early side effects are most noticeable.

Creamy and cheesy dishes. Carbonara, cheese-laden bakes and cream sauces are rich and dense. A smaller portion with vegetables is usually easier.

Alcohol alongside rich food. There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and emptier stomach can hit harder than usual, and alcohol adds calories that work against your goals.13 Moderation is the sensible rule.

Strong coffee on an empty stomach. There is no interaction between Mounjaro and caffeine, but strong coffee on a slower-emptying stomach can aggravate nausea or reflux for some people. High-calorie coffee drinks are also an easy place for the calorie deficit to leak.

06 · What to eat instead

What to eat instead of greasy food

Prioritise protein at each meal, vegetables and fibre, and regular fluids. That is the simple pattern behind most of the advice on eating well during treatment.13 You do not need a perfect food list. The aim is a sustainable calorie deficit you can keep up.

Different cooking methods. Baking, grilling, steaming, poaching and air-frying give you familiar food without it sitting in oil. Many favourite dishes work well this way.

Smaller, more frequent portions. Because the stomach empties slowly, a modest plate eaten slowly often feels better than a large one. Put your fork down between mouthfuls and notice when you are full.

Vegetables and fibre. They add volume and help with constipation, a common effect of treatment. Drink water with them.

Fat in sensible amounts. Fat is not the enemy. Oily fish, nuts, olive oil and avocado are not banned, but they are calorie-dense and still rich. Measure portions rather than guessing, and see how you feel.

Protein matters more as the dose rises and appetite falls. Some of any rapid weight loss comes from lean tissue, and eating protein at every meal alongside resistance exercise helps protect muscle.13 If appetite is low, a smaller protein-led meal beats a large greasy one.

07 · Fluids and bowels

Fluids, diarrhoea and constipation

Fatty meals can upset the bowels as well as the stomach. Diarrhoea and constipation are both among the most common effects of Mounjaro, so a greasy meal can tip you either way.13

Drinking enough matters more on Mounjaro than usual. Reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly. Dehydration is the main route to the rare kidney problems reported with this class of medicine. Keep water within reach through the day.

If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away; do not push through. Tell them if vomiting or diarrhoea lasts.

For constipation, fibre-rich vegetables, fruit, wholegrains and regular fluids help, along with gentle movement. A pharmacist can advise on anything over the counter, and you should check with them before starting any new product.

08 · Serious symptoms

When stomach pain after eating is not normal

Most digestive upset from a rich meal is unpleasant but short-lived. Some symptoms are different and need urgent attention. Serious side effects are uncommon, but you should seek immediate medical help for:113

  • severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis);
  • pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems);
  • signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing.

Get medical help straight away; do not wait to see whether it settles, and do not put severe pain down to a greasy dinner. If you take insulin or a sulfonylurea alongside Mounjaro, also watch for low blood sugar (shakiness, sweating, confusion), as your prescriber may need to adjust those medicines.

09 · Dose, timing and habits

How dose and timing change your tolerance

Everyone starts Mounjaro on 2.5mg weekly. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Because side effects are most likely when starting or moving up a dose, those are the weeks to be most careful with rich food. Keep meals simple, lighter and smaller while your body adjusts. Many people choose a weekend morning for their injection so that early side effects fall on days off.

Mounjaro is injected weekly, on the same day each week, at any time of day, with or without food.1 There is no best time of day, but what you eat afterwards can shape how the next day feels. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

Habits help too. Eat slowly, stop at comfortable fullness, avoid lying down straight after eating and keep movement gentle on difficult days.
