Mounjaro manages weight; it does not cure the biology behind it. That is the honest centre of this whole topic. Stopping suddenly is possible, but it does not mean the effects stop suddenly: the medicine stays in your body for weeks, then fades, and your appetite signals gradually go back to how they behaved before treatment.

If you are weighing this up, the main questions are practical. Why do you want to stop? Are you stopping because of side effects, cost, reaching a goal, or a life event such as pregnancy planning or surgery? The right route differs for each. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. 03 · What happens in your body

What happens in your body when you stop

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

That clearance period explains why the change is gradual rather than overnight. In the first weeks after your last injection there is still medicine in your system, so many people notice little at first. As levels fall, the effects the medicine was producing begin to fade.

Tirzepatide works by activating two natural gut-hormone receptors, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 When that signalling is no longer boosted, you should expect feeling full sooner and being less hungry between meals to become less pronounced.

Your patient information leaflet lists side effects and what to do if you feel unwell, and that remains your reference while the medicine clears.13 If you have type 2 diabetes or take other medicines, speak to your prescriber before stopping, because other treatments may need reviewing.

04 · Weight regain evidence

What the trial evidence says about weight regain

The clearest evidence comes from a trial built to answer exactly this question. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14

Those are averages, and your own result will differ. Some people regain more, some less, and habits built during treatment make a real difference. But the direction of travel is important: stopping did not leave people where they were, and continuing treatment kept moving weight downwards in that trial.

It also helps to see the starting picture. In the main weight-loss trial, people on tirzepatide alongside diet and exercise lost more than people on lifestyle changes alone, and these are trial averages where individual results vary.2 Part of that gap came from the medicine. When the medicine is withdrawn, the lifestyle changes alone have to hold the line.

This is not a reason to stay on treatment you do not want. It is a reason to prepare. Anyone stopping should expect regain to be a possibility, set a weighing routine, and agree with their prescriber what level of change would prompt a conversation.

Preparation starts well before the final pen, and its aim is to have habits doing the work that appetite suppression was doing. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.14

Practical steps worth agreeing with your prescriber:

  • Choose a planned last-dose date rather than drifting off when a pen runs out.
  • Write down the meals, portion habits and snacks that worked while your appetite was lower, and keep them going.
  • Decide how often you will weigh yourself and what change would make you contact the clinic.
  • Keep protein-containing meals and regular eating times in your routine, so hunger does not arrive unplanned.
  • Book a follow-up for the weeks after your last dose, when appetite changes become noticeable.

Exercise and habits that protect against regain

Regular activity is part of the licence, not an optional extra, and it matters even more once treatment ends. Nothing special is required: the evidence-backed pattern is regular moderate activity, such as brisk walking, plus resistance work for muscle.

Start where you are and build gradually. On days when you feel unwell, lighter movement still counts. Exercise also protects against regain if you later stop treatment, which makes the months while you are still on Mounjaro the best time to build the habit, rather than waiting until after the last dose.

Food habits matter in the same way. Treatment is used alongside a reduced-calorie diet and increased physical activity. A balanced pattern you can actually keep up is more useful than a strict plan you abandon within a few weeks.

Common reasons for stopping, and what to do for each

Side effects are one reason people stop, though most people stay on treatment. In SURMOUNT-1, 4.3% of people on 5mg, 7.1% on 10mg and 6.2% on 15mg stopped treatment because of side effects, compared with 2.6% on placebo.2 If side effects are the problem, tell your prescriber first, because a dose change may be an option before stopping altogether. You can also report them through the Yellow Card scheme.19

Mood is another reason. Regulators have looked at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee concluded in 2024 that the evidence does not support a causal link, and the MHRA keeps this under continuous review.17 Any big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.

Reaching your goal is a happy reason, but it is also the point where regain risk begins. If cost is the reason, talk to your clinical team about the regulated options. Short breaks, such as a holiday or illness, are best discussed with your prescriber, who will advise on timing of the next dose. Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
