How Mounjaro works, and why that matters for tolerance

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly, licensed in the UK for weight management in adults and for type 2 diabetes.1

Tirzepatide activates both GIP and GLP-1 receptors. Together these help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

That dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1 The practical point is that the effect on appetite depends on the medicine being in your system. It is not a switch that flips once and stays flipped.

04 · Dose escalation and adjustment

Dose escalation: why the first weeks feel different

Treatment begins on the 2.5mg pen and the dose steps up after at least 4 weeks, through 5mg, 7.5mg, 10mg and 12.5mg, to a maximum of 15mg, depending on how you respond and tolerate it.

The pens come in these strengths because the dose is meant to rise gradually.1 The starting dose is about letting your body adjust, not about weight loss itself.

05 · Plateaus and slower loss

Can you plateau on Mounjaro?

Yes, a pause in weight loss can happen on Mounjaro, and it does not by itself mean the medicine has failed. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 An average hides a wide range of individual experiences, so your own line on the chart will not look like the headline figure, and a stall for a few weeks is not the same as a stopped treatment.

A useful way to think about a plateau is as a prompt to check the basics with a clinician. Useful questions include whether your dose is still the one you were prescribed, whether any injections were late, whether your eating pattern has drifted, and whether activity has changed.

06 · Appetite returning

Why is Mounjaro no longer suppressing my appetite?

If your appetite feels stronger than it did, the first thing to do is tell your prescriber rather than assume the medicine has stopped working. The mechanism helps you feel fuller sooner and less hungry between meals, and it supports a reduced-calorie diet.1 It does not remove choice, and it works best alongside the diet and activity changes that go with it.

There are several everyday reasons appetite can feel different, and none of them requires a theory about tolerance. The injection day may have slipped, so the gap between doses has stretched. Your routine may have changed, with different sleep, stress or meal patterns. Your dose may be lower than the one that suited you best, or you may be mid-titration where the medicine is still being built up. These are questions, not conclusions, and a review is the way to answer them.

It is also worth separating hunger from habit. Some people find the physical sense of hunger remains quiet while cravings tied to routine, boredom or particular foods are still present. A prescriber or dietitian can help with that, and the medicine is intended to be one part of a wider plan.

07 · Breaks and restarting

Is it safe to restart Mounjaro after a break?

Whether it is safe to restart depends on how long you have been off and what dose you were on, and that is a decision for your prescriber. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

That slow clearance is one reason a short gap does not behave like a fresh start.

08 · Ozempic and Wegovy comparison

Does Ozempic eventually stop working?

Ozempic is semaglutide licensed for type 2 diabetes only; it is not a UK weight-loss medicine, and a change in effect is a reason for review with your prescriber. Semaglutide is a single-action GLP-1 medicine, whereas tirzepatide activates both GIP and GLP-1 receptors.1

Wegovy, also semaglutide, is dosed once weekly on a ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, so the 2.4mg maintenance dose starts from week 17. The 2.4mg dose is the licensed maintenance dose for weight management.18

People sometimes switch from one medicine to another when they feel the first has stopped doing enough. That is a clinical decision, and it should not be driven by online accounts or a guess about tolerance. 09 · Evidence and individual variation

What the trial evidence does and does not tell you

The headline trial for weight management gives averages and proportions, not promises. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

Read those figures for what they are. They show that most participants lost a meaningful amount, and they also leave a minority who lost less. Trial averages do not predict your own result, and a slow patch in any one person can sit comfortably inside the range of outcomes the trial reports.

For a worked example of scale only: for someone starting at 100 kg, 20.9% is about 21 kg. That is arithmetic on a trial average, not a forecast.

10 · Safety and reporting

Safety, new symptoms and the Yellow Card scheme

If something new appears while you are on Mounjaro, tell your prescriber, and consider reporting it through the MHRA Yellow Card scheme.19 This applies to symptoms you did not expect, to side effects that return after settling, and to anything that worries you.

If a symptom is not on the Mounjaro patient leaflet list, it is not a listed side effect; tell your prescriber about any new or lasting symptom.13

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

12 · NHS and private routes

NHS and private routes to a review

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
