Shedding is linked to rapid weight loss itself rather than a direct drug effect.1 In other words, the same thing can happen with any method that produces significant weight loss quickly, whether diet, surgery or medicine.

Tirzepatide works by activating two natural gut-hormone receptors, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties. In practice, most people feel fuller sooner and less hungry between meals. That helps you eat less, which is the point, but eating much less can also mean less protein and fewer nutrients reaching your body.

That is why the guidance stresses nutrition: protein intake and overall nutrition matter, which is another reason the reduced-calorie diet should stay balanced.1 A calorie reduction is intended; a nutritional gap is not.

How common is hair loss on Mounjaro?

It is common, but it is not the typical experience for everyone. Hair loss was reported by 4.9% of people taking tirzepatide in the weight-management trials, compared with 1.0% on placebo.1

Read that comparison carefully. Some people on placebo also reported hair loss, which tells you that hair changes happen for many reasons, including stress, illness, hormones and simply the normal cycle of hair. But the rate was clearly higher with tirzepatide, which is why it is listed as a side effect.

For context, the digestive effects are far more common than hair changes. Nausea, diarrhoea, vomiting, constipation and abdominal discomfort affect more people than hair shedding does, and they usually ease within days to a few weeks as the body adjusts.13

05 · Keeping hair healthy

How to keep hair healthy on Mounjaro

The most useful step is to keep your diet balanced while you eat less. Protein intake and overall nutrition matter.1 Treatment is meant to be used alongside a reduced-calorie diet and increased physical activity, not a crash diet.

Practical habits that fit with the guidance:

  • Build each small meal around a protein source, because with a smaller appetite protein is easy to skip.
  • Eat a varied mix of vegetables, wholegrains and fruit rather than relying on a handful of foods.
  • Keep gentle activity going, and include resistance exercise, which helps protect lean mass.
  • Stay hydrated, which also helps with constipation and tiredness.
  • Ask your prescriber about the pace of your loss. A steadier pace is a reasonable aim, not a failure.

If your appetite is so low that you struggle to eat enough, tell your clinical team. They can review your dose and hold your current strength for longer before stepping up, rather than pushing ahead.13

Variety and enough protein across the day matter more than any one food.

06 · Will it grow back?

Can hair grow back, and can you reverse it?

For most people, shedding linked to weight loss settles as weight stabilises.1

The sensible route is balanced nutrition with enough protein, a sustainable pace of loss, and patience while weight stabilises.

Do not stop treatment or change your dose on your own because of shedding. Speak to your clinical team first. If you do stop, tirzepatide has a half-life of about five days, so it takes roughly four to five weeks to clear the system, and nothing needs doing while it clears.1

If thinning continues after your weight has settled, the cause may be something else, such as a thyroid, iron or hormonal issue, or a hereditary pattern. That is worth raising with a GP or pharmacist.

Shampoo cleans hair and scalp; it is not a treatment for shedding caused by rapid weight loss. Hair-loss treatments exist for patterns such as hereditary thinning, but whether one is right for you depends on the cause, and a pharmacist or prescriber should assess that first.

Be wary of anyone promising specific results from supplements or "hair vitamin" products. These are low-cost supplements with no clinical oversight, and their dosing is not standardised. Tell your prescriber about anything you take so they can check it is suitable alongside Mounjaro.

When to speak to a clinician about hair changes

Speak to your prescriber or GP if the hair change does not look like general shedding. Sudden bald patches, a sore, red or flaky scalp, or hair loss alongside other new symptoms deserve a proper look, because they are not typical shedding.

Also get in touch if the shedding is distressing you, if it carries on after your weight has levelled off, or if you suspect you are not eating enough.

Seek urgent help for serious effects that have nothing to do with hair. Get medical help straight away for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.13

The same applies to loose skin, which commonly adapts gradually after weight stabilises. Protecting lean mass with adequate protein, resistance exercise and a loss pace guided by your prescriber is the counter-measure.

The EMA's safety committee reviewed the evidence in 2024 and concluded it does not support a causal link between GLP-1 medicines and low mood or suicidal thoughts, and the MHRA keeps this under continuous review. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.
