Can you take Mounjaro every 10 days instead of 7?
No, you should not move to a longer interval on your own. Mounjaro is a once-weekly medicine, and that is the only schedule the licence covers.1 Taking it every two weeks or "stretching" pens to save money is unlicensed, makes levels rise and fall unpredictably, and has no trial evidence for keeping weight off.
Wanting to inject every 10 days instead of 7 usually comes from a practical problem, not a medical one. Someone has a pen left over, a dose that makes them feel unwell, a bill they would like to spread out or a trip that clashes with injection day. Each of those has a better answer than quietly changing the interval.
Talk to the prescriber who looks after your treatment before changing anything. They can see your dose, your history and your goals.

03 · What the licence says
What the licence says about the dosing interval
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly.1 In the UK it is licensed for weight management in adults and for type 2 diabetes, and it comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
The important word is "once-weekly". The summary of product characteristics and the patient leaflet describe a weekly injection.113 None of them offers a longer-interval option for people who would like to inject less often.
If you realise you have injected late or missed an injection day, take the dose as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip the missed dose and take the next one on your normal day; never take two doses within 3 days of each other.
04 · Half-life explained
Half-life: why people think a longer gap might work
The idea that you could wait longer comes from a real property of the drug: it lingers. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.
Many people read that and reason, "if it stays in my body for weeks, surely I can inject less often." The logic is understandable, but it does not follow. A long half-life explains why the medicine does not vanish the moment a week is up. It does not tell you that a longer gap produces the same effect as the weekly schedule studied in trials.
05 · Risks of stretching
The risks of stretching the interval
Taking Mounjaro less often than weekly is unlicensed and makes levels rise and fall unpredictably.
Most side effects are digestive — the most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort — and they are most likely when starting treatment or moving up a dose.13
A muddled picture for your prescriber. If your real dosing pattern differs from your records, your prescriber cannot judge whether your dose is right. A plateau or a bad week then becomes impossible to interpret. Honesty about what you actually inject is more useful to them than a tidy log.
Return to your usual weekly day and tell your prescriber what happened. If you notice anything unusual, you can report it through the Yellow Card scheme.19
06 · Skipping and pausing
Skipping a week or pausing for a holiday
A skipped week is a different thing from a planned longer interval, but the answer is similar: ask before you improvise. If you miss an injection by accident, take it as soon as you can within 4 days (96 hours) of the scheduled time; if more than 4 days have passed, skip it and take the next one on your normal day, never taking two doses within 3 days of each other. Following that rule, and telling your prescriber, is the correct response.
Choosing to skip deliberately, for instance to "take a break on holiday", deserves more thought.
For travel, the simplest approach is to plan around it rather than skip. Check which day of the week you inject and whether the trip falls across it. Keep pens in a fridge at 2–8°C and never freeze them; once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away.1 Carry your pen in hand luggage, not the hold, and keep it in its original packaging. If you are unsure how your trip fits your schedule, message your prescriber before you go.
07 · Pen life and supply
Is it safe to use a pen for longer than planned?
A pen must be thrown away 30 days after first use, so stretching your doses is not a reason to keep using it. Once you start using a KwikPen, the UK product information allows it to be kept out of the fridge for up to 30 days (not above 30°C).1 That figure is about storage, not about how many injection days the pen will cover.
This matters for anyone who is stretching doses to make a pen last. If you inject less often, you may reach the end of the in-use period with medicine still in the pen. Do not use a pen that has been above 30°C or has been frozen, and throw it away after 30 days in use. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.13
If you are not sure how long your pen has been in use, or whether it is still safe, ask the pharmacy or check your patient leaflet rather than guessing. Never share a pen, and never use one that looks damaged, discoloured or has been left somewhere warm.

08 · Appetite and plateaus
Appetite returning, plateaus and "wearing off"
If your appetite seems to be creeping back, a longer gap is not the fix, and it is worth asking what else has changed.
In the SURMOUNT-1 trial of adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
Weight loss on Mounjaro is not linear; plateaus of a few weeks are normal and expected as the body adapts to a lower weight.
Before drawing conclusions, consider the things you can check with your prescriber:
- Whether you are on a dose that has had time to take effect.
- Whether your injection day and technique are consistent.
- Whether portions, snacking, alcohol, sleep and stress have crept up.
- Whether you have been unwell, or started another medicine.
- Whether you are building in enough activity alongside the injection.
Stopping is a different decision from stretching: in SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. 09 · Cost without stretching
Managing cost without changing the schedule
If money is the real reason you are asking, there are legitimate ways to bring the cost down that keep the schedule intact. If cost is the concern, the regulated options are moving to a lower licensed weekly dose or a prescriber-planned exit with diet and activity support; talk to your clinical team rather than self-adjusting.

The NHS is another route for people who qualify. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. 10 · Talking to your prescriber
How to raise this with your prescriber
Bring the real reason, not just the schedule question, and your prescriber can help far more. A message such as "I would like to inject less often because of cost" or "the day after my injection is hard" opens up options a bare question about intervals does not.
Useful things to say or send:
- Your current strength and how long you have been on it.
- Your usual injection day, and whether you have ever shifted it.
- Any days you have been late or have skipped, and what happened afterwards.
- Side effects, when they occur and how long they last.
- What you are trying to achieve and what is getting in the way.
Where your injection day itself is the problem, for example because it clashes with work or social plans, ask about moving it. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That is very different from lengthening the gap.

11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
