01 · The core answer

Can you take more than 15mg of Mounjaro?

No. Mounjaro (tirzepatide) is licensed in six strengths, and 15mg is the last of them. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 There is no seventh pen, no higher cartridge and no licensed way to exceed that top strength.

The licensed ladder is explicit about where it ends. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.111 The word that matters is maximum. It is a ceiling set by the licence, not a target everybody must reach.

Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1 If you are on a lower dose and wondering whether you ought to be pushing up, the answer is that your response and how well you tolerate the medicine decide it, not a comparison with other people.

What decision are you actually facing?

  • You are at 15mg and progress has slowed. This is the commonest reason for the question. The answer is a review with your prescriber, not extra medicine.
  • You have taken more than prescribed, or injected early. Talk to a doctor immediately and tell them exactly what you took and when. Do not wait to see how you feel; get medical help straight away if you are unwell.
  • You are thinking about it in advance. Good: the licensed ceiling is the plan. Focus on consistency, diet, activity and a dose you can live with.

It helps to separate what the medicine does from what it cannot do. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which help regulate appetite and slow how quickly the stomach empties.1 That action has a defined dose range in which it was studied and licensed. Beyond it there is no approved dosing instruction.

03 · Why not just go higher

Why more is not the answer

Exceeding the licensed dose means unknown accuracy, unknown tolerability and no regulator-endorsed guidance. Trying to engineer a larger dose, for example by combining pens or injecting more often, is outside the licence and is not something we or any UK regulator support.

04 · The licensed ceiling

Why Mounjaro only goes up to 15mg

Mounjaro stops at 15mg because that is the highest strength that was studied for the licence and approved for UK use. Medicines are licensed for a specific dose range. The range is defined by trial evidence on benefit and safety, then written into the product information that prescribers and pharmacists work from. For Mounjaro, the product information describes six strengths from 2.5mg to 15mg.1

That range is also what the pen is built to deliver. Mounjaro is supplied as a pre-filled multi-dose pen (KwikPen), and each pen contains four weekly doses.1 The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

This matters for the question you are asking. There is no mechanism on the pen to deliver something above the top strength, and no licensed product that does. Any suggestion that you can reach a higher dose by combining pens, topping up from another pen or injecting twice in a week is a workaround, and it carries all the risks of an unlicensed dose with none of the safety data.

Why the ceiling is not a failure

It is easy to read a maximum as an obstacle, but there is a more useful way to see it. The ladder was designed so that people can find the dose that works for them. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and your prescriber will guide the right pace.1 Reaching 15mg means you have used the whole range available. It does not mean the treatment has run out of road; it means the remaining questions are about consistency, lifestyle and whether this medicine is right for you.

How the ladder is meant to be used

The ladder is a guide to pace and tolerability. The 2.5mg starting dose is about letting your body adjust. Steps up happen at a minimum of four-week intervals, and depend on how you respond and tolerate it.

05 · What higher doses do

What higher doses of Mounjaro actually do

Higher licensed doses are associated with greater average weight loss in the trial data, but the gains are measured across the licensed range, not beyond it. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result. Some people respond strongly at a lower dose, and others need more time or a different approach.

The same trial reported how many people reached meaningful thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 The pattern is a gradual rise across the licensed strengths, not a cliff edge that a further increase would carry on up.

A worked example, kept honest

For someone starting at 100 kg, 20.9% is about 21 kg. That is the straight arithmetic of the trial average at 15mg, and it is not a prediction. It is also not a promise that stepping from a lower dose to 15mg will add a particular amount. Dose and result are linked on average, but your starting point, your diet, your activity, your sleep and your tolerance all shape what you see.

Beyond appetite

Higher doses do not simply mean stronger appetite suppression. 06 · If 15mg is not working

Why you may not be losing weight on 15mg

If progress has slowed at 15mg, the answer is a structured review, not a larger dose. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.1

There is also a formal checkpoint. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Notice the phrase highest tolerated dose. It is the dose you can sustain, which may or may not be 15mg.

Questions worth asking yourself first

  • Is the dosing consistent? The same day each week, with doses not skipped or delayed repeatedly. If you have missed doses, the missed-dose rule is to take it as soon as you can within 4 days (96 hours); after that, skip it and take the next on your normal day.
  • Has the trend really stalled? Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.
  • Is the diet still reduced-calorie, and is activity still increased? The medicine works alongside both, not instead of them.

What your prescriber can do

Your prescriber can review your response and discuss whether continuing or stopping is right for you.

Plateaus are a normal part of long-term weight management and they do not mean the medicine has stopped being the right tool. Trial results show a spread of outcomes even at the top dose, with some people losing less than the average. That spread is why the response checkpoint exists.

What not to do

07 · If you took too much

What happens if you take more than 15mg or an extra dose

If you have taken more than prescribed, talk to a doctor immediately, and get medical help straight away if you feel unwell. Treat it as something to report, not something to ride out.

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1 Each KwikPen contains four weekly doses.1

Common ways people end up with an extra dose

  • Forgetting that you already injected this week and injecting again.
  • Trying to catch up after a missed dose by doubling.
  • Changing injection day without leaving enough time between doses.
  • Starting a new pen while another still has medicine in it and mixing up which is current.

All of these are avoidable with a simple routine: pick a day, note it in a calendar or the free GLPTrackr companion app, and write the date on each pen when you start it.

What to tell the prescriber or pharmacist

Be specific: which strength, how many injections, how long ago, and whether you take any other medicines, especially for diabetes. If you take insulin or sulfonylureas, doses of those may need adjusting because of low blood sugar risk, so tell the doctor about them.

Tell them about any symptom you notice afterwards. If you have a side effect you think is linked, you can also report it through the MHRA Yellow Card scheme.19 Reporting helps regulators monitor safety, and it does not replace getting advice about your own situation.

Do not try to counteract it

Do not skip future doses on your own without advice. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

08 · Timing and missed doses

Dose timing: late, early and changing day

Mounjaro is weekly, and the licence gives clear rules for late and early doses, so you never need to improvise.

Injecting late

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 So injecting a day late is within the rule: take it when you remember and keep your normal day afterwards. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

If a different day suits you better, change the day properly rather than drifting.

Changing your injection day

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That limit is the safety floor: it stops two doses landing too close together.

Taking smaller or more frequent doses

Some people ask whether it is safe to take a low dose more often than weekly. That is not licensed. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

Time of day

There is no best time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember; many people choose a weekend morning so early side effects fall on days off.

Where to inject

Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1

09 · Pen life and storage

Pen life: why 30 days (not above 30°C) matters

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use — useful for short trips — and must then be thrown away.113 Keep the pen cap on to protect it from light, and never use a pen that has been frozen.

Using the same pen for longer than a month

A pen contains four weekly doses, which fits within the 30-day in-use window when you inject on schedule.1 If a pen has been in use for more than 30 days, throw it away rather than finishing it; do not use a pen that has been above 30°C. Past that window, sterility and potency are no longer assured.

Travelling

You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens can be kept at a temperature not above 30°C for up to 30 days, so most holidays need no special arrangements; for hot climates use an insulated pouch. Unopened pens

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.1

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

11 · NHS, private and ordering

NHS, private treatment and how ordering works

Whether you are treated through the NHS or privately, the dose ceiling is the same: 15mg weekly. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
