04 · In-use pen storage

Storing a Mounjaro pen that is already in use

An in-use Mounjaro pen can live in the fridge or out of it, within limits. The guidance says that once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded.1 That gives you flexibility without removing the fridge as the preferred place.

In practice, many people keep the pen in the fridge door or on a shelf away from the back wall and take it out only when it is time to inject. Others keep the pen in a drawer at home because it is convenient. Either approach is within the labelled conditions provided the room stays below the stated temperature and the in-use limit is respected. A pen left on a sunny windowsill, in a car glovebox or next to a radiator is not within those conditions, so those spots are best avoided.

The cap matters too. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1 If you put the pen back in the fridge, keep it away from the freezer compartment and the cooling element at the back, where food can freeze. Pens that have been frozen must not be used, even if they thaw and look normal.

People often ask whether putting the pen back in the fridge gives you extra time. The 30 days is counted from first use, so the safest approach is to count from the day you first used it. Finally, remember that once the pen has reached the in-use limit it should be discarded, even if some medicine seems to remain. Use the sharps bin supplied with your order for the empty or expired pen, and never put it in household waste.

For an unopened pen, the rule is simple: keep it in the fridge until first use. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.1 The in-use allowance is a feature of a pen that has been started, not a general licence for unopened stock to sit on a shelf.

So what if an unopened pen was left out? Perhaps it sat on the kitchen counter after a delivery, or came out of the fridge during a clear-out. The guidance is to check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.1

When you contact the pharmacy, have the following ready so they can advise quickly: the dose strength, the batch number and expiry date printed on the box, roughly where and for how long the pen was out, and whether it could have been somewhere hot or cold. A pen that has frozen must not be used. The storage guidance is blunt on this: never freeze Mounjaro pens, and never use a pen that has been frozen.1 Our page on the freezer question covers this in more depth.

What about warmth? An in-use pen can be kept out of the fridge not above 30°C for up to 30 days. A pen that has been above 30°C must not be used. Do not use it; keep the pen and contact the pharmacy.

You can also check the pen itself. A genuine UK pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless.1 Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days, or above 30°C in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.12

Run through it before every injection:

  • Is the liquid clear and colourless?
  • Is it free from particles and cloudiness?
  • Is the expiry date still ahead of you?
  • Has the pen stayed within the in-use limit?
  • Do you have any reason to think it froze?

07 · Travelling with your pen

Travelling with Mounjaro and returning it to the fridge

Travel is the main reason people ask about returning a pen to the fridge, and the in-use allowance is designed for it. You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription.1

In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch.1 A short break, a weekend away or a business trip will usually sit well inside that window.

For long stays, plan the dose schedule before you leave. Work out which injection days fall during the trip and how many days the pen will have been in use by then. If the trip would take the pen beyond its in-use limit, speak to your prescriber about the timing of your next order rather than carrying a pen that will expire while you are away. Check destination rules too. Different countries have different requirements for travelling with prescription injectables, so check them before you fly.1 Carry the pen in its original packaging with the pharmacy label, and keep sharps and needles with you in line with airline rules. Never put the pen in hold luggage, and never pack it against ice packs where it could freeze.

Unpack and put the pen in the fridge, away from the freezer compartment, keep the cap on, and note when you began using it. When you return home, put it back in the fridge as normal.

If the pen is delayed in transit, left on a plane, or exposed to unexpected extremes, treat it as you would any pen with an unknown history: stop, keep it, and contact the pharmacy. Replacing a pen is almost always a better outcome than injecting from one you cannot trust.

Your pen's storage history starts before it reaches you. A genuine UK pen arrives cold-chain packaged, so the arrival box is part of the evidence that it has been handled properly.1 When your delivery arrives, the sensible routine is to open the box promptly, check the contents, and put the pens into the fridge.

09 · Making a pen last

Making a pen last: the 8-week and extra-time questions

People often ask how to make a Mounjaro pen last longer, or whether a pen can be used for eight weeks. The honest answer is that you cannot extend a pen beyond its in-use limit. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1

Each pen is designed to deliver the doses of a treatment period at your prescribed strength, once weekly. If you feel your dose is too strong, too weak or causing side effects, the right move is to talk to your prescriber, who can review the plan during your monthly review.

What you can do is avoid waste. Store the pen correctly, keep the cap on, protect it from light and heat, and keep a note of when you started it. Do not leave it somewhere it might freeze. If you travel, use insulation and plan timing so the pen is not stranded beyond its limit. These habits protect the doses you have paid for.

Another frequent question is whether you need to prime a Mounjaro pen every time. Follow the pen's instructions in the patient information leaflet, which describes exactly what to do before each injection, and ask your pharmacy if the steps are unclear.13

And what should you avoid while taking Mounjaro? For storage, avoid freezing, heat, light and using a pen past its limits. Do not rely on lists from social media. Report any new symptoms to your prescriber, and you can report side effects through the Yellow Card scheme.19

10 · About the medicine

Why storage matters for this medicine

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Because the pen holds several doses over weeks, it is stored, carried and used repeatedly, which is why handling questions come up so often.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 Treatment works alongside a reduced-calorie diet and increased physical activity, and individual results vary.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, so they do not predict your own result.2

Each strength is a separate pen, and each has its own in-use clock once you start it. Keep any unused pen in the fridge until you need it and do not mix up pens of different strengths.

If you stop treatment, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

On access, Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first.4 Whether you use the NHS or a private pharmacy, the storage rules are the same.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
