Can you go up to 5mg Mounjaro after a couple of weeks?

No. The licensed titration does not allow a move to 5mg after only a couple of weeks. Everyone begins on the lowest strength and stays there for a minimum period before any increase.111 Everyone starts Mounjaro on 2.5mg once a week.1

The step-up rule is also stated plainly: After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.111

What about after three or four weeks?

Three weeks is still short of the minimum, and four weeks is the earliest point at which a step-up becomes possible, not a deadline. Each KwikPen contains four weekly doses, which is why a pen of 2.5mg naturally takes you to the earliest decision point.1

So, should you move up to 5mg after four weeks? Usually that is the planned next step, and it is what the ladder is built around. But it depends on how you respond and tolerate the medicine. 04 · Why the minimum exists

Why the licence keeps you on 2.5mg first

The starting dose exists for tolerability. This starting dose is about letting your body adjust, not about weight loss itself.1 That changes how you should read the first weeks. If you are not seeing dramatic results on 2.5mg, nothing is wrong. The step is doing the job it was designed for.

The mechanism is part of the reason for caution. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

Mounjaro also stays in the body for a while. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

This is also why "after a couple of weeks" feels tempting yet does not work in practice. You are only just past your first few injections, and you have not yet seen how your appetite, digestion and energy settle at the starting strength.

05 · Starting on 5mg

What happens if you start Mounjaro on 5mg?

Starting on 5mg is not part of the licensed schedule, because everyone starts Mounjaro on 2.5mg once a week.1 A prescriber working within the licence will not begin you on 5mg, and a pharmacy that offers to skip the starting dose should make you cautious.

06 · Are higher doses better?

Higher doses produced larger average losses in the main trial, but that does not mean everyone should chase the top of the ladder. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.

The same trial also shows that 5mg is an effective dose in its own right. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Notice how close those proportions sit. The gap between strengths is real but it is not a reason to rush the early weeks.

Is 5mg more effective than 2.5mg? On the ladder, 2.5mg is the adjustment phase and losses there are usually modest, while most measurable progress builds from 5mg onward, month by month.1 That is exactly why 5mg is the planned next step, and why you reach it at the pace the licence allows.

Equally important: many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. 07 · Slow progress on 5mg

Why am I not losing weight on 5mg?

Slow progress on 5mg is common and usually reflects pace, not failure. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.

Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. In SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12.2 Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.

If your scale has stalled, plateaus of a few weeks are normal and expected as the body adapts to a lower weight. Within the licence, the responses are:

  • Review food intake honestly, because portions drift back up.
  • Protect protein and activity.
  • Check the basics, such as sleep and alcohol.
  • Discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.

A plateau is not a reason to double doses or add unlicensed products.1

08 · Speeding up results

How can you speed up weight loss on Mounjaro?

The honest answer is that the dose ladder sets the pace, and the safe levers are the ones around it: food, protein, activity, sleep and regular reviews. Raising the dose ahead of the licence is not a lever.

Why do some people seem to lose weight so fast? Experiences differ widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal.1 Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 Dramatic early drops that you see shared online are not a benchmark for your own course.

Beyond the scales, trials showed other changes. Weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they are why progress is worth judging by more than the number on the scale.

If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.

Finally, keep your habits steady. Pick a day and time you will remember, and stick to it. There is no "best" time of day, because Mounjaro works the same morning or evening, with or without food.1

09 · Doses and timing rules

Missed doses, injection days and in-use pens

Dose timing rules matter whenever you are managing a step-up, because mistakes here are the commonest way people end up with doses too close together. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

On the practical side, keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen.1 When you step up, you start a pen of the new strength, so check the number in the dose window before you inject.

NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1 If you are pregnant or planning a pregnancy, Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive.
