Mounjaro is a prescription-only medicine. It is supplied only after an online consultation reviewed by a GPhC-registered prescriber, and it is used alongside a reduced-calorie diet and increased physical activity. Individual results vary.

03 · Planning
Why one fixed three-month bundle often does not fit
A fixed bundle assumes your dose is known in advance, and with Mounjaro it usually is not. The 2.5mg starting weeks are an adjustment phase, and the dose you respond to may still be ahead of you.1 If side effects, a plateau or a pause change the plan, a prepaid stack of the wrong strengths becomes a problem rather than a saving.

05 · What a pen contains
What one Mounjaro pen contains, and why that sets the maths
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.
Each KwikPen is designed and licensed to deliver exactly four weekly doses.13 One pen is four weeks, so two pens cover eight weeks and three pens cover twelve weeks. Each pen is designed to deliver four weekly doses, so use it as designed and then dispose of it.
The strength printed on the pen is the weekly dose, not the total in the pen. Your three-month plan therefore depends on which strengths you are prescribed, not just on how many pens you hold.

It also helps to know how long a pen lasts once opened. Unopened pens are stored in the fridge until first use and keep until the printed expiry date. A pen in use can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. If you want to hold several pens, the unopened ones stay in the fridge; read the patient information leaflet and ask the pharmacy about any out-of-fridge question for unopened pens.13
This matters when people ask about three months of supply. Keeping multiple pens at home means safe fridge space, checking expiry dates and tracking which strength is next. 06 · Dose ladder over 12 weeks
The dose ladder: what the first twelve weeks usually look like
The first twelve weeks of Mounjaro normally cover three strengths, because treatment starts low and steps up. A standard start is 2.5mg for at least four weeks, then 5mg, then 7.5mg, depending on how you respond, with the prescriber deciding when to move up.1 That is the pattern our cost table uses, and it is the reason three months is rarely three identical pens.
The 2.5mg starting weeks are about adjustment rather than results. Some people notice appetite change and early loss in the first week; others feel little until a higher dose. Both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.
Stepping up is a clinical decision. Your prescriber considers how you tolerated the last dose, how your appetite has changed and your wider health. A step-up is not automatic, and some people stay on a dose for longer than four weeks. If that happens, a pen bought for the next strength sits unused while you need another of the current strength. This is the main practical risk of a pre-bought three-month stack.
The dose-response in the main trial shows why the ladder matters. In SURMOUNT-1, 2,539 adults taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Those figures were measured at 72 weeks, not twelve, so they are not a three-month forecast.

07 · Eligibility and review
Who can be prescribed Mounjaro, and who decides the quantity
Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples are high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.11
A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable. That same assessment determines how much is supplied. Quantity is a clinical decision, not something you choose from a menu, and a prescriber may supply a single pen to begin with so that your response can be checked before more is issued.

That review pattern matches the way response is judged. Your prescriber looks at whether treatment is working: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you.1 A long prepaid supply cuts across that decision point, since it commits you to product before the review has happened.
If you are on other medicines, tell the prescriber. Some, such as insulin or sulfonylureas, may need dose adjustment because of low blood sugar risk, and oral contraceptives can be less well absorbed because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Those conversations are easier when each order is reviewed in turn. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly; Mounjaro must not be used during pregnancy, and should be stopped at least one month before trying to conceive.
People often ask whether a larger order is cheaper. Rather than assume it is, compare the per-pen price at the strength you will actually use, and remember that the new-customer discount applies to a first order and the reorder discount to each later order, so ordering pen by pen does not forfeit them.
08 · Expect by three months
What to expect by the end of three months
By three months, most people are still on the lower rungs of the dose ladder, so losses are modest compared with headline trial figures. In SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12.2 A sensible three-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.
Slower early progress does not predict failure, because the dose you respond to may still be ahead of you. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase, and most measurable progress builds from 5mg onward, month by month. Think in months, not days, and judge progress by trend rather than individual weigh-ins.
As a worked example, for someone starting at 100 kg, the 20.9% trial average on 15mg over 72 weeks is about 21 kg. That figure belongs to the full trial period, not to three months, and individual results vary. It shows why a three-month purchase is a stage in a longer plan rather than a complete course.
Plateaus of a few weeks are normal as the body adapts to a lower weight. Within the licence, the responses are to review food intake honestly, since portions drift back up, protect protein and activity, check sleep and alcohol, and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum. A plateau is not a reason to double doses or add unlicensed products.
Weight is not the only measure. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises.

09 · NHS versus private
NHS or private: where three months of supply comes from
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years.
That is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. If you qualify on the NHS, your supply will be arranged by your NHS clinical team and issued according to their schedule, so the question of buying three months yourself does not arise in the same way. If you do not yet qualify, or do not want to wait, private prescribing is the regulated alternative.

10 · Buying safely
Buying safely: counterfeit risk and bulk purchases
The safest way to buy several pens is from a pharmacy you can verify on the GPhC register. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified.7 Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.
Only buy from a pharmacy you can find on the register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation. Anyone offering to supply without a consultation or prescription is a warning sign, however attractive a multi-month price looks. When your pens arrive, check them. A genuine pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been above 30°C. An in-use pen must be thrown away after 30 days out of the fridge (not above 30°C). If supply or packaging ever seems wrong, check the pharmacy on the register and contact the pharmacy before injecting.13

11 · No stretching or extras
Making a supply last: why stretching and extra doses are unsafe
Some people consider buying a larger supply and spacing it out to save money. That is unlicensed and not safe: it makes blood levels rise and fall unpredictably. Mounjaro is licensed as a once-weekly medicine, and its blood levels are designed around a 7-day cycle.
Talk to your clinical team rather than self-adjusting.
Each KwikPen is designed and licensed to deliver exactly four weekly doses. Use the pen as designed, then dispose of it. Needles, a sharps bin and injection guidance are included with every order.13
Understanding the half-life also helps if you stop. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 If you pause for any reason, tell your prescriber before restarting, because the dose you restart on is a clinical decision.

12 · Year cost planning
Planning beyond three months: what a year costs
A first year of Mounjaro is 52 weeks, or 13 pens, and the total depends on the dose you settle at. At standard prices, before discounts, a first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. A new customer's first order is £40 less, and every later order is £25 less.
For other maintenance doses, use the monthly price: £279.99 at 7.5mg, £290.99 at 10mg and £319.99 at 12.5mg. These are per-pen prices before the reorder discount, so a three-month stage at one of those doses is three of that pen, with each later order £25 off.

The point of the table is that three months is a stage, not an endpoint. Whether to continue is a conversation to have with your prescriber. In the SURMOUNT-4 trial, continued treatment was studied for maintaining weight reduction, which is why stopping and the plan for after it deserve thought at the start.14
