How should you come off Mounjaro?

The safest approach is to agree a plan with your prescriber before your last dose. Mounjaro manages weight; it does not cure the biology behind it. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.14

You will often see advice to "taper" or "wean off" gradually. Your prescriber sets the plan for you, based on your dose, your progress and your reason for stopping.

02 · Stopping suddenly

Can you stop Mounjaro cold turkey?

You can end treatment by simply not taking the next dose, but we advise planning it with your prescriber first. The reason is regain, not the drug leaving your system. After a final dose, tirzepatide takes roughly 4–5 weeks to clear, and there is no way to speed this up.1

03 · If cost is the reason

If cost is why you want to stop

Cost is one of the most common reasons people look at coming off, and there is a regulated alternative to stopping outright.

At standard prices, the 5mg Mounjaro pen is £218.99 per month and 7.5mg is £279.99. A first year costs £2,711.88 if you settle at 5mg, up to £3,599.88 at 15mg. What happens when you stop Mounjaro?

Weight regain is the main thing to plan for. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 These are trial averages and individual results vary.

As a worked example, for someone at 100 kg after treatment, 14% is about 14 kg. That is arithmetic on a trial average, not a prediction for you.

Tirzepatide works on two gut-hormone receptors, GIP and GLP-1. Together they help regulate appetite and slow how quickly the stomach empties.1 Most people feel fuller sooner and less hungry between meals while on treatment. When treatment ends and the medicine clears, that support goes with it.

05 · Build habits first

Build your habits before the last dose

Start the habit work while you are still on treatment, because that is when it is easiest. Increased physical activity is part of the licence, not an optional extra.1 The evidence-backed pattern is regular moderate activity, such as brisk walking, plus resistance work for muscle. Start where you are and build. On injection days with early side effects, lighter movement still counts. Exercise also protects against regain if you later stop treatment.

Mounjaro is used alongside a reduced-calorie diet and increased physical activity.1 The practical task before an exit is making that diet and activity routine something you can keep up without the medicine. Useful steps include:

  • Agree your timing and support with your prescriber.
  • Keep a regular activity routine, with some resistance work.
  • Keep a regular eating pattern you can sustain without the medicine.

06 · What not to do

What not to do when coming off

Mounjaro is licensed as a once-weekly medicine, and its blood levels are designed around a 7-day cycle.

Avoid using a pen beyond its in-use life. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or frozen. Unopened pens stay in the fridge until first use and keep until the printed expiry date.13 After that point, sterility and potency are no longer assured.

07 · Stopping for side effects

Stopping because of side effects

If side effects are why you want to stop, speak to your prescriber before deciding. In SURMOUNT-1, 4.3% of people on 5mg, 7.1% on 10mg and 6.2% on 15mg stopped treatment because of side effects, compared with 2.6% on placebo.2 So stopping for this reason is not unusual, and your prescriber can discuss your options.

Report any suspected side effect, including new symptoms, through the MHRA Yellow Card scheme.19 Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts. If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night.

08 · Who should be reviewing you

NHS and private support for coming off

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 If you are NHS-treated, your NHS team should plan any exit.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1 If you are on the contraceptive pill, Mounjaro can reduce how well it is absorbed: switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive.

Once in use, a UK Mounjaro KwikPen is licensed for up to 30 days (not above 30°C) out of the fridge. Beyond that window, sterility and potency are no longer assured, so the pen should not be used. Unopened pens stay in the fridge until first use and keep until the printed expiry date.
Can you move to a lower dose instead of stopping Mounjaro?
