Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. You do not need a laboratory explanation to recognise that the old approach is not working as it did. The practical point is that hormonal and metabolic change can make diet-only approaches slower.
It also helps to separate causes. Poor sleep, low mood, stress and reduced activity commonly travel with menopause, and each can nudge appetite and routine. None of this is a personal failing. It is a reason to look at the whole picture rather than blaming willpower.

05 · Mounjaro and HRT
Can you take Mounjaro with HRT?
Yes, Mounjaro can be prescribed alongside HRT, and there is no known interaction between them.1 That is the most common practical worry we hear from women in perimenopause and menopause, so it deserves a direct answer.
There is one point to be aware of. Oral medicines in general can be absorbed more slowly when you start treatment, because the medicine slows how quickly the stomach empties.1 If your HRT is a tablet, mention that to your prescriber so they can factor it in. If you use patches, gels or sprays, still list them, because your prescriber needs the full picture.
Oral contraception is a related topic. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. This applies to the pill only — implants, coils and injections are unaffected. Women still using contraception during perimenopause should raise this at consultation, and the patient information leaflet explains the details.13 Perimenopause does not automatically mean pregnancy is impossible. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
Tell your prescriber everything you take, including HRT, over-the-counter products and supplements. This matters more than people expect. Herbal products marketed for menopause or weight can contain active ingredients, and your prescriber cannot advise on what they do not know about.
Eligibility is the same at any adult age. Age is not a barrier and menopause is not a barrier. The licensed criteria are the criteria, and your prescriber assesses you against them.
If you take medicines for other conditions, such as diabetes treatments, say so clearly. Doses of some of them may need adjusting, so this is a conversation to have with your prescriber rather than something to manage yourself.
It is also worth telling your regular GP or menopause clinic that you are starting treatment, even though that is your choice. Joined-up care tends to work better, particularly if your HRT dose is being reviewed at the same time.

06 · Natural suppressants
Natural appetite suppressants during menopause: an honest look
Natural appetite suppressants are widely marketed to women in menopause, but none carries the regulatory status of a licensed weight-management medicine. They are low-cost supplements with no clinical oversight, and that is the single most important fact about them.
The marketing often leans on phrases such as nature's Ozempic, and on dramatic weight-loss claims. A supplement being natural says nothing about whether it works or is safe for you, and herbal products can interact with other medicines you take.
For berberine and similar products, there is no comparable trial evidence. If a page tells you a precise amount of weight you will lose, treat that as a warning sign. Dosing is not standardised across products, so two bottles with the same label can differ.
Fibre and filling supplements are a slightly different case. They are available cheaply without assessment and may help some people feel full, but they are not weight-loss medicines and are not a replacement for a prescriber's review. None of this means that food choices are pointless. Eating patterns that keep you satisfied, regular meals, protein and fibre-rich foods, sleep and movement all matter, and none requires a product. They are the foundation that licensed medicine is used alongside.

07 · NHS or private
NHS or private: how access works
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
For a woman in menopause, this means your NHS route depends on your BMI and your health conditions, not on your age or menopause status. Ask your GP whether you fall into a group currently being prioritised in your area. Local pathways can differ, and your GP will know what is available near you.
If you do not yet qualify on the NHS, or do not want to wait, regulated private treatment is an alternative. It is not a shortcut around assessment. A GPhC-registered prescriber still reviews your suitability, and the licence and safety information are the same.
Wegovy sits in a different place. NHS access to Wegovy runs through specialist weight-management services; ask your GP.

08 · Safety and side effects
Safety, side effects and what to tell your prescriber
Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity. Individual results vary. This page is information, not a substitute for medical advice.
The full list of side effects is in the patient information leaflet.13 Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.
A symptom that is not on the leaflet's list is not a listed side effect. If something new appears after you start, discuss it with your prescriber and report it through the Yellow Card scheme.19 Menopause itself produces many symptoms, including changes in mood, sleep and libido, so it can be hard to tell what is causing what. Your prescriber can help you work that out.
Menopause itself can affect sexual desire. If libido is a concern, raise it with your GP or menopause clinician, since there are treatments aimed at it.
If you stop treatment, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1
Storage matters too. Keep unopened pens in the fridge at 2–8°C until first use, and they keep until the printed expiry date. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. For anything beyond that, such as unopened pens left out of the fridge, check the patient information leaflet and ask the pharmacy.13

How Mounjaro compares with other licensed options
Mounjaro is not the only licensed medicine that acts on appetite, and the right choice depends on your health, preferences and prescriber's advice.
Tirzepatide activates both GIP and GLP-1 receptors. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide, the active ingredient in Wegovy and Ozempic.1
Wegovy's pens run through the dose ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly. The dose steps up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, so the 2.4mg maintenance dose starts from week 17. The 2.4mg dose is the licensed maintenance dose for weight management.18
Liraglutide, sold as Saxenda and in products such as Erly, is supplied at a concentration of 6mg per millilitre. It is a daily injection, unlike once-weekly Mounjaro and Wegovy. Some people find a weekly schedule easier to remember, others prefer a more familiar routine. That is a personal decision for you and your prescriber.
Tablet options exist too, including Wegovy tablets and Foundayo.

11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
