04 · What Mounjaro is

What Mounjaro is and how it works

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Each strength is a separate pen, and each pen contains four weekly doses, so one pen lasts four weeks.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy). Both are licensed, effective treatments, and the right choice depends on your health, your preferences and your prescriber's advice. Wegovy is also a once-weekly injection, with a dose ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after at least four weeks until the 2.4mg maintenance dose from week 17. Liraglutide products, such as Saxenda, are different again: a daily injection at 6mg per millilitre.

Tirzepatide has a half-life of about five days. After a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That matters if you are thinking about stopping, switching or planning around an event: the effect fades gradually rather than overnight.

Whether you walk into a clinic or order online, a prescriber must assess you first. 05 · Who is eligible

Who can have weight loss injections

Mounjaro is licensed for weight management in adults with a BMI of 30 or above. It is also licensed for adults with a BMI between 27 and 30 who have at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity.

Those are the licensed criteria, and they are the starting point rather than the full decision. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.11 That is why the consultation asks about existing conditions and everything you take, including over-the-counter products. Answer it fully and accurately: the questions exist to protect you, and a complete picture lets the prescriber give a safe decision.

If your BMI sits just below the threshold, or you have a condition you are unsure counts, the consultation is the place to find out. It is free and carries no obligation. 06 · NHS and private

NHS or private: the honest picture

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If you live in Kings Hill, the sensible first step is a conversation with your GP about whether you fall into a current NHS group. NHS treatment is free at the point of use for people who qualify, apart from any usual prescription charge, which is £9.90 per item in England for those who pay. 07 · Realistic results

What results to expect, realistically

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 As a worked example only, for someone starting at 100 kg, 20.9% is about 21 kg.

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend rather than individual weigh-ins.1

The starting weeks differ widely between people. Some notice appetite change and early loss in week one, while others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

Your response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you. 08 · Doses and yearly cost

Dose ladder and what a year costs

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.

Because cost depends on the dose you reach and stay on, here is how planning looks. All figures are standard prices before discounts. A new customer's first order is £40 less, and every later order is £25 less.

09 · How to inject

How to inject, and what to do if you slip

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.113

There is no "best" time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off, and then stick to it.

The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window. Follow the pen's instructions for the injection itself, and read the leaflet in the box before your first dose.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That is useful if your routine changes, for example after a shift pattern change or a trip. Make the move once and then keep the new day consistent.

10 · Storage and travel

Storing and travelling with your pens

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, which is useful for short trips, and must then be thrown away. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.113

Unopened pens should be kept in the fridge until first use, and they keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, and the UK pen is licensed differently. You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.1

11 · Food, activity and alcohol

Eating, protein, exercise and alcohol on treatment

No food is banned on Mounjaro, including bananas, but some choices make treatment easier and more effective. Prioritise protein at each meal, vegetables and fibre, and regular fluids. Many people feel worse after very fatty, fried or very sugary food. Large portions sit heavily because the stomach empties more slowly, so smaller, slower meals work better. Treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit, not a perfect food list.1

Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance. The two levers are adequate protein and resistance exercise two or more times a week. Aim to include a protein source at every meal, which is harder when appetite is low. This matters more, not less, as the dose rises and appetite falls.

Increased physical activity is part of the licence, not an optional extra. Nothing special is required: the evidence-backed pattern is regular moderate activity, such as brisk walking, plus resistance work for muscle. Start where you are and build. On injection days with early side effects, lighter movement still counts. Exercise also protects against regain if you later stop treatment. Kings Hill has plenty of paths and open space for walking, and a few resistance sessions at home or a gym are enough to begin.

There is no direct interaction between Mounjaro and alcohol. However, drinking on a reduced appetite and emptier stomach can hit harder than usual, and alcohol adds calories that work against your goals. If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar, and doses of those medicines may need adjusting, which is a conversation for your prescriber or GP. Moderation is the sensible rule, and many people find their desire for alcohol drops on treatment.

Practical habits help in the first weeks. Eat slowly and stop when you feel full. Keep a protein-rich option ready for days when appetite is very low. Sip fluids through the day. If nausea is troublesome, smaller meals and avoiding greasy food are the first things to try, and your prescriber can advise if it persists. Tell the prescribing team about any new or worrying symptom rather than waiting, because the monthly reviews are there for exactly that.

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Raise this at your consultation so your prescriber can advise on what suits you.7

12 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition, such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and more activity. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you.1
How much weight can I expect to lose?
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide with diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, versus 3.1% with lifestyle changes alone. These are trial averages and individual results vary. Progress is gradual, so think in months, not days, and judge it by trend rather than single weigh-ins.2
What is the Mounjaro dose schedule?
Everyone starts on 2.5mg once a week, which is about letting your body adjust rather than weight loss itself. After at least 4 weeks the dose rises to 5mg, then can step up by 2.5mg at minimum 4-week intervals through 7.5mg, 10mg and 12.5mg to a maximum of 15mg weekly, depending on response and tolerance. Many people settle at 5mg, 10mg or 15mg, and higher is not automatically better.1
