Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. What to expect when you take Mounjaro 7.5mg

Expect a gradual change, not a sudden one. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 By the time you reach 7.5mg you have already been through that adjustment, so you have seen how your body responds to the medicine.

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 Most side effects are digestive. The most common — affecting more than 1 in 10 people — are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1

Trials measured results at 72 weeks, so think in months, not days.1 Trial averages do not predict your own result, and slower early progress does not predict failure, because the dose you respond to may still be ahead of you.

Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4

Get medical help straight away for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis. Pain in the upper right abdomen, fever or yellowing of the skin or eyes could signal gallbladder problems, so get medical help straight away. Swelling of the face or throat and difficulty breathing are signs of a severe allergic reaction: seek immediate medical help or call 999.1 Tell your prescriber about other new or lasting symptoms, and you can report suspected side effects through the Yellow Card scheme.19

The licensed answer is that you do not start there, because everyone starts Mounjaro on 2.5mg once a week. There is no single dose that works for everybody, and the amount you need is the dose you respond to and tolerate. The ladder runs up to a maximum of 15mg weekly, but many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.1

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

The same trial reports how many people crossed useful thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Again, individual results vary.

So how much do you need? Enough to get a response you can sustain, with side effects you can manage. Is 2.5mg enough? It is a starter dose designed for adjustment, so most measurable progress builds from 5mg onward.1 Is 5mg enough? Do you need a GLP-1 medicine to lose weight?

A GLP-1 type medicine is a prescribing option, not a requirement, and a prescriber decides whether it suits you. Mounjaro is used alongside a reduced-calorie diet and increased physical activity, not instead of them.1 In SURMOUNT-1, lifestyle changes alone produced an average loss of 3.1% over 72 weeks, compared with 15% on 5mg, 19.5% on 10mg and 20.9% on 15mg of tirzepatide. Individual results vary.2

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Beyond the scales, weight loss on tirzepatide in trials came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.1

09 · Plateaus and stepping up

Within the licence, the responses are: review food intake honestly because portions drift back up, protect protein and activity, check the basics such as sleep and alcohol, and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.1 A plateau is not a reason to double doses or add unlicensed products.

10 · Injecting and storing

Injecting, missed doses and storage at 7.5mg

The practical routine is the same at every strength. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.1

There is no best time of day. Mounjaro works the same morning or evening, so what matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.1

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, which is useful for short trips, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen. Keep the pen cap on to protect it from light.1

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. For hot climates use an insulated pouch, and check destination rules for travelling with prescription injectables before you fly. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.1

11 · Supply and safety

Genuine supply, cost context and safety

The 7.5mg pen is £279.99 per month at the standard price. For context, across the UK pharmacies we track, Mounjaro pens ranged from £143.96 to £339.00 per month across all doses as of 11 October 2026. The gap between providers for the same dose ranged from £27 to £41. Compare like for like, at the same dose, and check what is included.

On supply, we use official LillyDirect supply, so pens come direct from Eli Lilly's UK distribution as genuine KwikPens. Counterfeit medicines are a real risk when buying injectables from unregulated sellers. The MHRA publishes counterfeit medicine alerts, and anyone offering to supply Mounjaro without a consultation or prescription should be avoided.7 You can check our registration on the GPhC register (9012878).

Serious side effects are uncommon but need urgent attention. Get medical help straight away for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis), or for pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems). Signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing, need immediate medical help.1

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. This applies to the pill only — implants, coils and injections are unaffected.17 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive.1 If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion), because your prescriber may need to adjust those medicines. Report any suspected side effect through the Yellow Card scheme.19

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1
