Why Sildenafil and Viagra Are the Same Medicine at Very Different Prices

Sildenafil is the active substance in Viagra. Not similar to it, not a version of it. The same substance.

That is not a marketing claim. It is what UK regulation requires before a generic can be licensed at all. A generic medicinal product must have the same active substance as the reference medicinal product, must have the same pharmaceutical form, and must have been shown to be bioequivalent to the reference medicinal product.

So the honest framing is not which is better. It is: the same active ingredient, at the same strength, in the same form, at prices that differ by several times over.

This page explains where that gap comes from, and why some men report preferring the branded version. Because licensed generics must demonstrate bioequivalence, there should not be a clinically meaningful difference in how the medicine works, although individual experiences can still vary.

Is Sildenafil the Same as Viagra?

The Same Active Ingredient

Yes. Sildenafil citrate is what does the work in both, and both work the same way. Viagra is a brand name for a product whose active substance is sildenafil, and generic sildenafil is that same substance sold under the name of the substance itself.

This is the normal relationship between a brand and a generic across all of medicine. Paracetamol is sold under brand names and as paracetamol. The relationship here is the same one.

So whichever way round you searched it, you are comparing a branded sildenafil product with a generic sildenafil product containing the same active substance.

How They Work

The mechanism belongs to the substance, so it is identical in the branded product and in every licensed generic.

Sildenafil citrate inhibits an enzyme called phosphodiesterase type 5, or PDE5, which is found in the erectile tissue. Blocking PDE5 allows the smooth muscle in the local blood vessels to relax, and that relaxation lets blood flow into the tissue more easily. Improved blood flow is what restores erectile function in men whose difficulty is circulatory.

The tablet does not work on its own, because the body's own natural signalling has to be present first. Both require arousal to be effective, and neither causes an automatic erection without it.

The same action on blood vessels explains the second licensed use of the substance. In pulmonary arterial hypertension, relaxing the blood vessels in the lungs lowers the pressure the right side of the heart works against. Those products contain the same active ingredient at different strengths, under different brand names, and are prescribed for an entirely different purpose.

What the Regulator Requires of a Generic

For a generic medicine to be licensed in the UK it must satisfy three requirements. It has the same active substance as the reference medicinal product. It has the same pharmaceutical form. And it has been shown to be bioequivalent to it.

Bioequivalent means the generic delivers the active substance into the bloodstream in a way that matches the original closely enough that no clinically meaningful difference is expected. That has to be demonstrated with studies before a licence is granted.

Why the Price Differs So Much

Patent Expiry and Generic Competition

The price gap exists because of how medicine development is funded, and it is not evidence of a quality difference.

A company that develops a new medicine holds patent protection for a period. During that time it is the only supplier, and the price reflects the cost of development, the cost of the failures that never reached market, and the absence of competition.

Viagra was first introduced in the UK in 1998, as the first oral treatment for erectile dysfunction. Once its patent protection expired, other manufacturers could apply to license their own versions. To do that they must meet the generic requirements above, which is a substantially cheaper route than a full application, because the original evidence package already established that the substance works.

The result is competition, and competition drives the price down. Three things follow, and all three are worth knowing:

• The gap is largest immediately after generics arrive and narrows over time as the branded price responds
• Generic prices keep moving, because they track how many manufacturers are supplying at a given moment
• A large gap is normal, not a warning sign

The Manufacturer Difference

The branded product comes from the originator. Generic sildenafil is made by a range of manufacturers, which is precisely why it costs less. Most generic sildenafil products are sold under the name of the substance rather than under a brand name of their own.

Every manufacturer supplying a licensed medicine in the UK must meet the same quality standards for manufacturing, regardless of whether the product carries a brand name. The licence sets the standard, not the name on the box.

The Cost Difference

Two cautions on reading that.

Compare per tablet, not per pack. These are advertised starting prices for the listed packs, not necessarily equivalent per-tablet prices. Because pack sizes can differ, compare the total number of tablets and calculate the price per tablet before drawing conclusions.

Check the live figure. Prices change, and a price displayed after a discount code has been applied is not the standard price. Every figure here is a standard price at the time of writing.

The comparison comes down to the size of that gap rather than to the exact figures on any given day.

Does Generic Sildenafil Feel Different?

What Bioequivalence Does and Does Not Promise

Bioequivalence promises that the active substance reaches the bloodstream in a comparable way. It does not promise that two people, or the same person on two occasions, will have identical experiences.

Individual variation in this medicine is documented and large. The mean absolute oral bioavailability of sildenafil is 41%, with a range of 25 to 63%. That spread exists between people taking the same product.

Other documented factors move the outcome more than branding does:

• Food. On an empty stomach the medicine reaches its peak sooner. A meal delays peak concentration by around 60 minutes and reduces it by around 29%
• Alcohol. Excessive alcohol can temporarily reduce the ability to achieve an erection
• How many attempts. In the studies, 40.8% of men reported improvement after the first dose, rising to 78.4% across repeated attempts up to eight doses
• Grapefruit juice. Documented as able to affect how sildenafil works

Tablet Appearance

Generic tablets often look different from the branded original, and they are permitted to.

Appearance is not part of what bioequivalence controls. Shape, colour, size, markings and coating can all differ between manufacturers, and generic sildenafil tablets from two different manufacturers may not look the same as each other either.

If you are re-supplied and the tablets look different from last time, that usually means the pharmacy has sourced from a different licensed manufacturer. It does not mean you have been given something else. If you are ever unsure, the dispensing label and the patient information leaflet in the pack identify what it is, and the pharmacy can confirm.

The Placebo Finding

This is the most interesting question on the page, and it has a real number attached.

In fixed-dose clinical trials, 25% of men receiving placebo reported improved erections. That demonstrates that reported outcomes in these trials can be influenced by factors other than the active medicine.

Expectation genuinely affects outcomes here. If a quarter of men improve on a tablet containing nothing, belief and confidence are doing measurable work. A person's expectations and previous experiences may influence how they perceive their treatment.

That is not the same as the branded tablet being pharmacologically superior. The active ingredient is identical and bioequivalence has been demonstrated. What differs is what the person brings to it.

Why Some Men Prefer the Brand

Beyond the expectation effect, three ordinary explanations cover most cases.

Familiarity. The brand name has been known for decades. The substance name is unfamiliar to most people, and unfamiliar is not reassuring when something matters to you.

Assumed hierarchy. People reasonably assume price tracks quality, because in most purchases it does. Medicines are an exception, because a regulator sets the standard both products must meet.

Attribution. A disappointing evening on the generic gets blamed on the generic. A disappointing evening on the brand gets blamed on tiredness, stress or alcohol.

None of that makes anyone foolish. It makes it a personal preference rather than a pharmacological finding, which is fair to know when deciding what to spend.

Is Generic Sildenafil as Reliable?

On the regulatory standard, yes. A licensed generic must contain the same active substance, in the same pharmaceutical form, and must have been shown bioequivalent to the original before it can be sold.

Reliability of effect rests on the licence, and the licence requires equivalence.

Reliability of supply rests on where you buy from. A licensed generic from a GPhC registered pharmacy is a known quantity. A tablet from an unregulated seller is not, whatever it is labelled as, and that is a far more important distinction than brand versus generic. MHRA guidance is direct on the point: where medicines come from unverified sources, there is no guarantee they are safe or effective.

Strength, Onset and Duration

Strengths Are the Same

The strengths available are the same, because they belong to the substance rather than to the brand. Sildenafil is licensed in three strengths in the UK, and the branded product is licensed in the same three, so a given strength of one corresponds exactly to the same strength of the other.

A generic is licensed at a strength that matches the reference product, and its bioequivalence was demonstrated at that strength.

This has a practical consequence. There is no conversion involved in moving between them, because there is nothing to convert. The same dose delivers the same amount of active substance. That is different from switching between two different medicines, where the strengths are not equivalent.

Which strength suits you is a clinical decision either way. Your prescriber will determine the appropriate treatment, and you should stay with the strength on your dispensing label rather than adjusting it yourself.

No Onset Difference

There is no documented onset difference, because onset is a property of the substance and bioequivalence covers exactly this.

Peak plasma concentrations are reached within 30 to 120 minutes, with a median of 60 minutes. Sildenafil at the standard strength was documented as effective at various times post dose, from less than 1 hour to up to 4 hours after administration. Those are the figures for both.

If one seems slower than the other on a given evening, the documented explanations are food, alcohol, grapefruit juice and individual variation rather than the brand. A heavy meal alone shifts the peak by around an hour.

No Duration Difference

Also no documented difference, for the same reason.

The terminal phase half-life of sildenafil is 3 to 5 hours, and the documented window runs from less than 1 hour to up to 4 hours after the dose. Those figures describe the substance, and there is no mechanism by which the name on the packet would extend or shorten them.

Prescription Requirements

What an assessment covers is the same regardless of which you end up with:

1. Your medical history, including heart disease, any previous heart attack, blood pressure and any other underlying health conditions

People move this way for the reasons in the preference section above. What we would not do is tell you the branded product works better, because the evidence does not support it and it would be a straightforward way to sell you something more expensive.

Side Effects and Safety

Side effects belong to the substance as well, so the profile does not change with the name on the box.

Headache is listed as very common in the product information. Facial flushing, indigestion, nasal congestion, dizziness and visual disturbances such as blurred vision or a temporary change in colour vision are listed as common. These are usually mild and usually short lived, and they are listed for the branded product and for every licensed generic because they were documented for the substance itself.

A smaller group needs attention rather than tolerance. A prolonged erection lasting longer than 4 hours needs urgent medical attention. Sudden loss of vision or hearing, chest pain or fainting means stopping and seeking medical advice straight away.

Men with heart disease, a recent heart attack or a recent stroke, and men with other underlying conditions affecting blood pressure, need assessment before any treatment is supplied rather than afterwards. If you take other medicines, the interaction list applies identically to both, because it belongs to the substance.

Safety Points That Apply to Both

• Never take more than one tablet in any 24-hour period
• Do not take extra tablets or combine doses to increase the strength unless your prescriber specifically tells you to
• Never combine sildenafil with another medicine in the same class
• Neither may be taken with nitrates, nitric oxide donors, nicorandil, amyl nitrite or riociguat
• Stay with the strength and quantity on your dispensing label
