Agreement

  • I have answered all consultation questions truthfully and understand that incorrect answers may affect my health.
  • I understand this medication must be combined with a healthy diet and increased physical activity.
  • I will not take any other weight loss medication at the same time, and I will use this treatment for personal use only.
  • I will inform my GP of any changes in medication or symptoms, and I accept responsibility for informing healthcare professionals about this treatment before any surgical procedure.
  • I understand that this medication is not suitable if I am trying to conceive within the next 2 months.
  • I will stop this treatment and seek medical help immediately if I experience severe abdominal pain, neck swelling, or changes in mood.
  • I understand the prescriber may decide not to issue treatment based on my consultation responses.
  • I understand that I must provide a prescription or evidence of treatment from an alternative provider (such as my GP or another pharmacy) if this is my first order and I am requesting a dose higher than the starter dose, or if more than 8 weeks have passed since my last order and I am increasing my dose.
  • I give permission for EverydayMeds to access my NHS Summary Care Record or request medical history from my GP if required, and I consent to relevant consultation details being shared with my GP to ensure continuity of care.
