Major Drug Interactions with Sildenafil
Dangerous Interactions - Never Combine
The most serious sildenafil interactions occur with nitrate medications used for chest pain. These include glyceryl trinitrate (GTN spray), isosorbide mononitrate, and isosorbide dinitrate. When taken together, these combinations can cause severe, potentially fatal drops in blood pressure. This interaction can occur even if nitrates are taken several hours apart from sildenafil, as the ED medication remains active in the body for 4-6 hours.
Riociguat, a medication for pulmonary arterial hypertension, is another absolute contraindication. Like sildenafil, riociguat affects blood vessel dilation, and combining them creates unpredictable blood pressure effects.
Alpha-Blockers and Blood Pressure Medications
Alpha-blockers such as doxazosin, tamsulosin, and alfuzosin (commonly prescribed for prostate enlargement) can cause significant blood pressure drops when combined with sildenafil. While not absolutely contraindicated, these combinations require careful monitoring and often dose adjustments. Patients should be stable on alpha-blocker therapy before starting sildenafil, typically beginning with the lowest 25mg dose.
Other blood pressure medications including ACE inhibitors, beta-blockers, and calcium channel blockers may enhance the blood pressure-lowering effects of sildenafil, though these interactions are generally manageable with appropriate monitoring.
Enzyme Inhibitors and Liver Metabolism
Certain medications significantly affect how the liver processes sildenafil, leading to increased drug levels and enhanced effects. Strong CYP3A4 inhibitors include ketoconazole and itraconazole (antifungal medications), ritonavir and saquinavir (HIV treatments), and clarithromycin and erythromycin (antibiotics). When these medications are necessary, sildenafil doses typically need reduction to 25mg, and the 100mg dose should be avoided.
Grapefruit juice can also inhibit these same liver enzymes, potentially increasing sildenafil levels, though this interaction is generally mild compared to prescription medications.
Other Important Interactions
Bosentan, used for pulmonary hypertension, may reduce sildenafil effectiveness by increasing its breakdown in the liver. St. John's Wort, an herbal supplement for depression, can similarly reduce sildenafil levels and effectiveness.
Alcohol doesn't directly interact with sildenafil but can worsen side effects like dizziness and low blood pressure. Excessive alcohol consumption can also impair erectile function, reducing treatment effectiveness.
Safe Medication Management
Before starting sildenafil treatment, whether choosing generic sildenafil or branded Viagra (both options are available at EverydayMeds), patients must provide complete medication lists during consultation. This includes prescription medications, over-the-counter drugs, and herbal supplements. Never stop prescribed medications without consulting your doctor - instead, discuss alternative ED treatments or adjusted dosing schedules with healthcare providers.
Always inform healthcare providers about sildenafil use before starting new medications, and seek immediate medical attention if experiencing chest pain, severe dizziness, or fainting after taking sildenafil with any other medication.
