Clinical Information & Treatment Guidelines
What is Promethazine?
Promethazine hydrochloride, commonly known by the brand name Phenergan, belongs to the phenothiazine class of sedating antihistamines. Unlike newer non-sedating antihistamines, promethazine crosses the blood-brain barrier, producing drowsiness that can be therapeutically beneficial for certain conditions.
Approved Uses
The BNF lists promethazine for multiple indications including allergic rhinitis (hay fever), urticaria (hives), motion sickness, nausea and vomiting, and short-term management of sleep disturbances. Its dual action as both an antihistamine and mild sedative makes it particularly useful when allergies interfere with sleep quality.
How Promethazine Works
Promethazine blocks histamine H1 receptors throughout the body, preventing the allergic cascade that causes symptoms like sneezing, itching, and runny nose. Its anticholinergic properties contribute to anti-nausea effects, whilst its ability to cross into the central nervous system produces the sedating effect that helps with sleep difficulties.
Dosage Guidelines
For allergic conditions, adults typically take 25mg at night or 10-20mg two to three times daily. When used for sleep difficulties, 25-50mg at bedtime may be appropriate for short-term use only. EverydayMeds stocks Phenergan 25mg tablets in 56-tablet packs and Phenergan Elixir 5mg/5ml 100ml bottles for flexible dosing options.
Important Safety Considerations
Promethazine should not be used in children under 6 years due to respiratory depression risk. The sedating effects can impair driving and operating machinery. Alcohol should be avoided as it increases drowsiness. Elderly patients may be more sensitive to anticholinergic side effects including confusion and urinary retention.
Available Formulations
EverydayMeds offers both tablet and liquid formulations. The tablets provide convenient dosing for adults, whilst the elixir allows for more precise dose adjustments and is often preferred by those who have difficulty swallowing tablets. Both formulations contain the same active ingredient with equivalent therapeutic effects.
