The most important thing about a first dose is that it is a poor predictor of the eventual result.

There is a second figure worth holding alongside it. In those same studies, 25% of patients on placebo reported that treatment improved their erections. Expectation does measurable work here, in both directions.

What the Evidence Says, Instead of Reviews

Individual accounts are not comparable, because bioavailability varies from 25 to 63% between people and outcomes vary with the underlying cause. The trial data is more useful, and it is specific:

  • 9,570 patients received sildenafil across 74 double-blind placebo-controlled studies
  • 40.8% reported improvement after the first dose, rising to 78.4% across repeated attempts
  • 84% of men whose difficulty was psychological in origin reported improvement
  • 59% of men with diabetes reported improvement
  • 25% of men on placebo reported improvement

That last pair is the honest picture. The cause of the difficulty changes what is likely to happen more than the brand or the seller does.

How to Take It

How to take it is set by your dispensing label and by the patient information leaflet in the pack, not by a web page. Follow those.

What a page can usefully do is set out the safety guardrails that apply regardless of what has been prescribed:

  • Never take more than one tablet in any 24-hour period

Grapefruit or grapefruit juice may affect sildenafil exposure, so check the product information or ask your pharmacist whether you should avoid it. Everyday meals are not a problem in themselves. This is a matter of timing rather than diet.

With Alcohol

The direct answer is documented and specific. Sildenafil 50mg did not worsen the blood-pressure-lowering effect of alcohol in healthy volunteers with mean maximum blood alcohol levels of 80mg per dl, meaning that at that level the combination did not lower blood pressure more than either would alone.

That finding is narrower than it looks. It measured blood pressure, not outcome: excessive alcohol can temporarily reduce the ability to achieve and maintain an erection, entirely separately from any blood pressure interaction. And 80mg per dl is a specific level, saying nothing about heavier drinking.

Side Effects

Headache is listed as very common, affecting more than 1 in 10 people. Facial flushing, indigestion, nasal congestion, dizziness and visual disturbances such as blurred vision or a change in colour perception are listed as common, affecting up to 1 in 10.

Many common side effects are mild and temporary, but persistent or troublesome symptoms should be discussed with a healthcare professional.

Effects That Need Urgent Attention

  • An erection lasting longer than 4 hours. Urgent, as delay can cause lasting damage
  • Chest pain during or after exertion. Never treat this with a nitrate if you have taken sildenafil
  • A sudden decrease or loss of vision, in one or both eyes
  • A sudden decrease or loss of hearing
  • Fainting or collapse
  • Signs of an allergic reaction

Who Cannot Take It

Some conditions and medicines are contraindications, and mean sildenafil should not be prescribed.

  • Nitrates and nitric oxide donors in any form, including sprays used only occasionally
  • Nicorandil, amyl nitrite and riociguat
  • Low blood pressure, and uncontrolled high blood pressure
  • Recent stroke or recent heart attack
  • Severe liver impairment
  • Known hereditary degenerative retinal disorders
  • Previous loss of vision in one eye from a condition affecting the optic nerve

Other factors change how much medicine reaches the bloodstream rather than ruling it out. In severe kidney impairment exposure roughly doubles, and in cirrhosis it rises by around 85%. Certain antibiotics and antifungals raise levels substantially, with erythromycin increasing exposure by 182%.

Alpha blockers, often taken for blood pressure or prostate symptoms, are not an absolute exclusion. The product information states that patients should be stabilised on alpha blocker therapy before sildenafil treatment is started.
